What causes it

Weight is not a simple matter of effort. It is regulated by a system that includes appetite signalling, metabolic rate, sleep, stress, medication, and genetics, and that system defends a set point more stubbornly than most people are led to expect.

This is why weight lost through effort alone is so often regained. The body responds to loss by raising hunger signals and lowering the rate at which it burns energy. That response is physiological rather than a failure of will, and it is the reason medical support changes the picture for many patients.
