Why Botox Does Not Treat Every Wrinkle

Botox relaxes muscle. That is the whole of what it does, and it explains both why it works so well on some lines and why it does nothing at all for others.

The lines Botox does not reach

Static lines. A dynamic line appears when you move and disappears when you stop. A static line sits there at rest, etched into the skin by years of sun and collagen loss. Relaxing the muscle underneath softens it slightly at best. The line lives in the skin, so the skin is what has to change.

Volume loss. Cheeks, temples, and lips lose structure with age. The result reads as sagging, but nothing about it is muscular, and no neuromodulator will replace what is gone.

Texture and tone. Pigment, pore size, and scarring are surface concerns. Botox has no mechanism for any of them.

What pairs with it

Filler, for structure. Botox settles the movement, filler restores what was supporting the tissue. Different problems in the same face.

Lasers, for the skin itself. Fractional resurfacing and broadband light work on pigment, texture, and static lines, and they build collagen, which is the thing Botox cannot do.

Why this matters at consultation

Patients sometimes arrive asking for Botox for a concern Botox cannot address, then leave disappointed by a product that did exactly what it was supposed to. A provider's job is to say which of your lines will answer to a neuromodulator and which will not, before anything is injected.
